Frontpath Health Coalition offers a large selection of PPO providers in the Toledo area. At the Frontpath site, you can search for a provider by specialty, location or other variables. If your preferred physician is not part of the Frontpath network, you can do several things.
Let us know! We will contact the provider and encourage a review of the benefits to joining the Frontpath PPO network. Email us at the Benefits Office.
Let your doctor know the Plan is now using the Frontpath network and that you would like him to join! Patients are often a strong factor in a doctor’s choice to join, or not join, a particular network.
You may continue to see non-Frontpath providers, however you will be responsible for additional out-of-pocket expenses if the charges exceed the Plan’s Usual, Customary and Reasonable determination.

Our managed care partner who reviews inpatient and outpatient medical care has a website. The Plan offers HCS's MaterniCare program for expectant mothers and fathers. The Plan requires enrollment in the program within 30 days of pregnancy diagnosis. A website is provided to participants featuring a scheduler/ reminder calendar, exercise/ activity log, a nutritional analysis, and other online resources. Once enrolled, participants can utilize the MaterniCare website.

The Plan contracts with MHN to manage our mental health and substance abuse benefits. The MHN website will allow you to locate network providers. You may also call MHN at 800-472-4992, 24/7. All treatments must be pre-approved by MHN.
Putnam 401(k)

401(k) participants can now access their Putnam investment information online. Just go to www.ibenefitcenter.com
You will be asked for your user ID and password, which you should have received in the mail from Putnam. If you missed that mailing, or have any other questions regarding your account, you can call Putnam directly at 877-UNION-44.

Effective 1/1/2004, Active Participants and Retirees receive prescription coverage through Restat. You may also call 800-248-1062 for information.

The Plan contracts with Vision Service Plan (VSP) to bring you discounted services for vision care. The VSP website will allow you to locate network providers, determine when you are eligible for benefits and also tell you the Plan’s benefits for network and non-network providers.
